To be used for a teacher-initiated removal
Pursuant to Section 37.002(b), Texas Education Code, I am removing __________________ from my class for the following reason(s). The student has (check applicable reasons):
_____ been documented by me to have repeatedly interfered with my ability to communicate effectively with the students in the class or with the ability of the student's classmates to learn. Copies of that documentation are attached.

A copy of the disciplinary report of that behavior is attached.
